BwD Sports Council will judge all applications supplied, and may request more supporting information, however their decision will be final.

 

All sections of the application form must be completed within the strict word limit (excluding any contact details) please enter N/A were any sections are found not to apply to the nominee.

 

Any information supplied in relation to your entry may be used by the Council for event publicity, and forwarded as a northwest nomination through Active Lancashire Awards, please clearly mark any sensitive details and we will keep in strictest confidence.
